New Owner Never Registered Vehicle

Hi there,

I purchased a vehicle for my ex, who took the vehicle with her when we separated. She has never moved the vehicle on to her name and it's still on my name.

How do I go about getting it taken off my name and/or put into her name?

She has not attended to this despite numerous requests for her to do so.
